Sekworm is first encountered during the quest Test Monkey. It has the ability to instantly regenerate to full health when its health is low, therefore is very difficult to kill. Since it prefers to keep its distance, it's easier to walk around it and complete the quest without killing it. Keep in mind Emperor Gulkar will be waiting for you when you get out of the hellrift, so it's best not to waste too much resources to try to kill Sekworm.
